Cheri is the Director and owner of SOAR Counseling Services, Inc. since 1993. She has 20+ years of experience working with adults who struggle with chronic and persistent mental illness, substance abuse, anxiety, depression, family conflicts, at-risk adolescents, self-esteem, relationship, and abuse issues.
She holds a Master’s Degree in Rehabilitation Counseling from the University of Wisconsin—Madison, 1993 and practiced in Community Support Programs & Assertive Community Treatment(ACT) for 6 years and 5 years in County administration positions learning how the system works. Soar Counseling Services, Inc. is a privately owned outpatient mental health and addiction clinic. In the fall of 2014 I began providing ACT services in Park County. A wholistic, mindfulness approach addressing spirit, mind, body, with being solution focused.

I had found my calling in life, helping people with severe and persistant mental health issues and destigmitization of mental illness.
Our services are only reimbursed by medicaid at this time. I am currently working on alternative resources for individuals on medicare to become eligible for medicaid.
I also work with individuals through the disability process.

What is Assertive Community Treatment?
*Team of professional working together to support persons with high needs in the community who have a mental illness
*Clients need to be committed to building relationships with their team. The services are flexibile to the clients need. We might see someone 3 times a day or 1 time a month depending on need. We want to be seemless so people dont fall through the cracks and information is accurately passed on throughout the indivials care providers. Working with the individual to be assertive and have the ability to communicate effectivly to assure highest quality of care.
*Case loads of 10/12 therapist/casemanager.
*Medicaid billed services only.
*Each client has one person who is their primary therapist and then that therapist coordinates with other team members to assist in different areas of individuals life ie: vocational, medical, social. So each client has a team who supports them with their goals which is set every 6 months.
*All services are voluntary so each client can choose which services they want and can leave the program at anytime for any reason.
*Areas that are addressed include social, family, vocational, medical, spiritaul, legal, nutriational, educational, financial, residential, recreational.
*Primary Goal is for individuals to stay in the community and live independently reaching their highest potential.
*Groups to include recreational, spiritual, vocational, art, social, educational and equine (when available).
*We can provide payee services when needed with the goal of the client becoming as independent as possible when possible.
